A randomized trial of methadone and buprenorphine for treatment of opioid dependence.
Opioid substitution treatment (OST) with sublingual methadone is the recommended first line treatment for opioid dependence. The current study compared the safety and efficacy of a new patient-controlled sublingual administration of buprenorphine (Temgesic) to the standard daily methadone. Forty-six patients from a methadone maintenance program were randomly assigned to buprenorphine or methadone. The two groups were similar in demographics and history of opioid use. The course of opioid dependence was monitored by buprenorphine dosage requirements, opioid withdrawal symptoms, and rates of retention. The cost of treatment was assessed from direct costs, indirect costs, and savings in criminal justice and welfare costs. Results were analyzed using Mann-Whitney tests and Fisher exact tests. Forty-six patients were enrolled, including 25 in the buprenorphine group and 21 in the methadone group. No differences in demographics, opioid use, or course of opioid dependence were found. The mean dosage at the end of treatment was 10 mg of buprenorphine twice daily for both groups. The treatment costs per patient were $8,600 for buprenorphine and $11,600 for methadone. There were no differences in drug side effects or opioid withdrawal or in rates of retention between the two groups.
