To evaluate the efficacy of medical thoracoscopy (MT) in diagnosing and treating tuberculosis (TB) and malignancies of the lung. Patients were divided into two groups. The first group consisted of 30 patients diagnosed as having pulmonary TB with a lung lesion found by MT. The second group consisted of 20 patients with lung lesion revealed by open thoracotomy. Diagnosis was confirmed by bronchoscopy, smear, culture, and pathology. Patients were treated surgically or endoscopically. The patients were followed up, and efficacy was assessed on the basis of clinical manifestations, treatment, and the results of CT scan and pathology. All patients in group 1 were cured. In group 2, surgical resection was performed, and 14 were cured, and 3 underwent operation twice for recurrent primary tumors. There were two deaths in group 2. MT is a safe method to establish a precise diagnosis of lung TB and primary lung malignancy. MT is superior to routine x-ray in diagnosing TB and identifying the part of the lesion.
