Hepatitis C virus (HCV) remains the leading cause of end-stage liver disease and liver transplantation. The treatment of HCV infection with hepatotropic nucleoside analogues may lead to histopathologic damage in the transplanted liver but may lead to sustained virologic responses. There is a paucity of data regarding the effects of chronic HCV infection in the transplanted liver. To determine the effects of HCV infection on the transplanted liver in children, we reviewed the data from our liver transplant database. Between May 1988 and October 1994, 27 patients received 29 liver transplants for end-stage liver disease due to HCV. HCV infection was either associated with other chronic liver diseases (n = 10) or idiopathic (n = 19). Histology was reviewed by a single pathologist using morphometric analysis. The main outcome measures were the presence of well-differentiated hepatocytes and ductular reaction, as well as the percentage of hepatocytes with fibrosis and cirrhosis. In the 20 patients who survived > or = 12 months postoperatively, the median survival time was 24 months. At 1 year, 12 patients (60%) had no evidence of chronic rejection; at 2 years, this figure was 72% (95% confidence interval, 47-90%). One patient developed recurrent HCV and subsequently died. A lobular neutrophilic inflammation and a ductular reaction were not found in any patients. Forty-eight percent (14/29) of the transplants had evidence of moderate to severe fibrosis.
